Clomiphene citrate is a non-steroidal selective estrogen receptor modulator (SERM). It is a pharmaceutical drug that was FDA approved in 1967 to treat infertility in women. Its brand name is Clomid.

Although not FDA approved for this use, it is commonly and safely prescribed to men to treat low testosterone also known as hypogonadism.

Primary hypogonadism is a hormonal pattern in which the testes do not produce desired levels of testosterone.  This pattern of low testosterone is typically seen in aging males, although it can also occur in younger males.

Secondary Hypogonadism is a hormonal pattern in which the brain (hypothalamus or pituitary gland) is not sending appropriate signals to the testes, which results in a failure to produce adequate testosterone levels. This pattern is more commonly seen in younger males, likely due to a combination of head trauma, genetics, environmental factors and lifestyle choices (poor sleep, high stress, poor nutrition, lack of exercise) that can negatively impact one’s overall health.

Overview of Clomiphene for Men

In men, clomiphene selectively and temporarily blocks estrogen receptors in the brain which in turn stimulates the pituitary gland in the brain to produce luteinizing and follicle stimulating hormone.  This promotes testosterone and sperm production from the testes. When indicated, Wellcore clinicians prescribe this medication to naturally increase testosterone levels and fertility, particularly in younger men who want to have children in the future.

Clomiphene commonly increases a man’s testosterone levels by 1.5-2x.  With this, some men can experience an increase in libido, mood, strength,  muscle mass, and sexual function.

It can also be a valuable tool in transitioning someone off of testosterone replacement therapy (injections, creams, pellets) back to their age appropriate baseline testicular function.  This helps to avoid testosterone deficiency symptoms with the abrupt cessation of testosterone replacement therapy.

Benefits of Clomiphene for Men

Multiple clinical studies in the last 30 years have demonstrated the safety and effectiveness of Clomiphene at improving testosterone levels and fertility in men.

One of the advantages of using Clomiphene over other hormone therapies is its tolerability and covenience. It is an oral medication typically taken daily or every other day.

Side Effects and Risks of Taking Clomiphene

Although it is well tolerated by most, common side effects include bloating and stomach pain.  Less common side effects are headache and mood and visual changes which resolve with cessation of the drug.

Recommended Dosage for Taking Clomiphene for Men

The recommended dosage for taking Clomiphene for men is crucial for the effectiveness and safety of the treatment. Your healthcare provider will prescribe Clomiphene with clear instructions on proper usage and dosage.

The recommended Clomiphene dosage for men is 25mg - 50mg daily or every other day, depending on the individual's needs and response to treatment.

The appropriate dosage may differ based on individual circumstances.

Other Treatments for Male Infertility

While Clomiphene is a popular and effective treatment for male infertility, alternative treatments may also be helpful. These options can include lifestyle changes such as improved sleep, nutrition, exercise, stress management or supplements.

However, other methods are still available when these alternative treatments do not work.

Medications

Medications to address underlying medical conditions such as diabetes, thyroid problems, and other hormonal imbalances may also be prescribed to help with male infertility.

Surgery

Another option to treat male infertility is surgery. Surgical treatments can include varicocelectomy, which is the surgical repair of enlarged veins in the scrotum. It also can include epididymal sperm aspiration, a procedure that removes sperm containing fluid from optimal areas of the epididymis.

Other procedures, such as testicular biopsy or ejaculatory duct reconstruction, may also be necessary, depending on the cause of infertility.

In Vitro Fertilization (IVF)

In Vitro Fertilization is a procedure involving reproductive technology. It helps couples conceive a baby struggling with infertility through sexual intercourse.

The procedure involves retrieving eggs from the female partner and fertilizing them in a laboratory. Once this stage is complete, the embryo is implanted into the female partner's uterus.

IVF has helped many couples successfully become pregnant and have healthy babies.

Artificial Insemination

Artificial insemination is another option for couples — specifically, those struggling with infertility. This process entails the direct transfer of sperm from the male partner to the female partner's uterus.

The procedure of artificial insemination can be done via a donor sperm or with the couple's own sperm.

What Other Drugs Affect Clomiphene

Clomiphene, as with any medication, interacts with other drugs and could cause complications if not taken as prescribed. Individuals prescribed Clomiphene should be informed about the possibility of drug interactions.

It could be dangerous to take other medications while on Clomiphene without alerting your healthcare provider first.

Ensure you get advice from medical professionals or your doctor before taking Clomiphene alongside any existing medication.
